# The "Silent" Killer: Suspension Trauma and Your Rescue Plan

In the world of industrial and construction safety, a Personal Fall Arrest System (PFAS) is a miracle of engineering. It is the last line of defense between a worker and a catastrophic ground impact. We spend hours training on harness inspection, lanyard anchorage, and fall clearance calculations. Yet, there is a dangerous complacency surrounding what happens *after* the arrest.

We focus so much on stopping the fall that we often overlook the threat that begins the moment the fall stops: **Suspension Trauma**, or more accurately, **orthostatic intolerance.**

Too many job sites treat the mandatory "rescue plan" as a box to be checked on a safety compliance form - a "paper exercise" destined to sit in a binder. But when a worker is dangling in a harness, the clock is not just ticking; it is screaming.

## **What is Suspension Trauma?**

When a person falls and is held upright by a harness, gravity takes over in a way the body isn't designed to handle. The leg straps of a harness compress the femoral arteries. Because the worker is suspended in a vertical position with limited movement, blood begins to pool in the lower extremities.

This venous pooling reduces the amount of oxygenated blood returning to the heart and brain. Within minutes - sometimes as few as 10 to 15 - a worker can experience dizziness, nausea, and loss of consciousness. If left suspended, the condition can lead to kidney failure, cardiac arrest, or death.

This is why, in the world of high-angle safety, we don't call this a "waiting game." We call it a medical emergency that requires immediate intervention.

## **The Myth of "Help is on the Way"**

A fatal flaw in many site-specific safety plans is the reliance on local emergency services. Many site managers operate under the assumption that if a fall occurs, they will call 911, and a fire department with a ladder truck or technical rescue team will arrive within minutes.

### **This is a dangerous miscalculation.**

Even in urban environments with rapid response times, the logistics of a technical rescue are complex. Emergency responders must arrive, assess the scene, set up specialized rigging, and execute the rescue. By the time they have made contact with the victim, the "critical window" for suspension trauma may have already closed.

OSHA 1910.151 mandates the provision for "prompt rescue" in the event of a workplace injury. Relying on an outside agency whose response time is variable does not meet the spirit of "prompt."

## **ANSI Z359.4: Your Blueprint for Action**

The ANSI Z359.4 standard provides the framework for Safety-at-Height programs, specifically regarding assisted-rescue and self-rescue systems. The standard is clear: **The responsibility for rescue lies with the employer, not the municipal fire department.**

To ensure true compliance and, more importantly, human survival, companies must transition from "paper rescue plans" to "pre-engineered rescue systems."

### **Why Pre-Engineered Kits are Non-Negotiable**

1. **Speed of Deployment:** Pre-engineered kits - such as those featuring remote-attachment capabilities - allow on-site workers to reach the suspended victim without needing to climb into the hazard zone themselves.
2. **Standardization:** When an emergency happens, adrenaline spikes and decision-making slows. A pre-engineered system simplifies the process, reducing the risk of rigging errors during the heat of the moment.
3. **Site-Specific Readiness:** By keeping a rescue kit staged at the point of work, the "rescue team" becomes the crew already standing on the platform. They are trained, equipped, and seconds away from the victim.

## **The Takeaway: Stop "Papering" Your Safety**

If your rescue plan hasn't been practiced in the last six months, it isn't a rescue plan - it’s a liability.

Don't wait for a tragedy to realize that "prompt rescue" is the most vital component of your fall protection program. Invest in pre-engineered kits, train your team to use them, and move beyond the compliance binder. Your workers aren't just trusting their harnesses to save their lives; they are trusting you to bring them back down to earth before the "silent killer" sets in.
