In the high-stakes environments of industrial and construction work, the Personal Fall Arrest System (PFAS) is the final line of defense between a worker and a catastrophic incident. Yet, there is a dangerous epidemic sweeping across jobsites: inspection complacency.
We have all seen it - the rapid-fire, five-second scan of a harness. A worker glances at the straps, gives the buckles a quick tug, and clips in. While this satisfies the bare minimum of a "visual check," it fails the fundamental requirement of safety: identifying the degradation that lives beneath the surface.
To truly protect lives, we must move beyond the superficial and adopt the rigorous standards set by OSHA 1926.502(d)(21) and the ANSI Z359.2 framework.
The Invisible Threat: Why "Visual" Isn't Enough
The most dangerous flaws in fall protection equipment are often the ones you cannot see at a glance. Complacency leads to a false sense of security, where workers assume that because a harness looks clean, it is safe.
1. Internal Fiber Fraying
Webbing is composed of thousands of synthetic fibers. Over time, friction, exposure to the elements, and repeated loading cause internal abrasion and general degradation. While the exterior might appear uniform, the inner core of the strap can be compromised. If your webbing feels "limp" or has a different consistency compared to a new harness, it is likely suffering from internal fatigue.
2. UV Degradation
Synthetic fibers are susceptible to ultraviolet radiation. Prolonged exposure to the sun breaks down the chemical bonds in the material. A harness that has spent months in the back of a truck or hanging on an exposed rack may look perfectly fine, but the webbing could be brittle and lose up to 50% of its tensile strength.
The Components: Beyond the Harness
A PFAS is a system, not just a product. According to ANSI Z359.2, every component requires meticulous scrutiny:

Anchorage Connectors: Check for deformation, corrosion, or pitting. If an anchorage point has been subjected to a fall arrest force, it must be removed from service immediately, even if it looks untouched.

Connecting Devices (Lanyards & SRLs): Check for smooth retraction and the integrity of the locking mechanism. On Self-Retracting Lifelines (SRLs), check for "nicks" or "burrs" on the cable that could slice through the housing during a lock-up.
On lanyards, look at the condition of the energy absorber and the leg component (whether it be webbing, rope or wire rope) and look for signs of wear and tear. Inspect the mechanical function and condition of connectors (snap hooks and carabiners).

Full Body Harness: Focus on the stitching. Look for pulled threads, color changes (which indicate chemical or UV exposure), and the condition of the D-ring(s) and buckles or adjustors.
The Competent Person: Documentation and Authority
Compliance is not just about the check - it is about the record. OSHA and ANSI stress the role of the Competent Person, someone capable of identifying hazards and who has the authority to take prompt corrective measures.
Documenting the Inspection
If it isn’t written down, it didn’t happen. A robust inspection log should include:

The Unique Identifier: Every piece of gear should have a serialized tag or RFID.

The Criteria: Document specific observations (e.g., "no evidence of chemical exposure," "stitching integrity confirmed").

The Decision: Clearly state "Pass" or "Fail/Retired."
When to Retire: The "Newness" Fallacy
A common mistake is keeping equipment in service simply because it hasn't reached its manufacturer’s expiration date or because it "looks brand new."
Equipment must be officially retired if:

It has arrested a fall: There is no "re-certification" for a harness that has caught a falling body.

Chemical Exposure: Fading, discoloration, or a "crusty" texture are clear signs that chemicals have compromised the integrity of the fibers.

Heat Damage: If the equipment was near welding sparks or high-heat environments, the fibers may have melted, charred or shrunk, even if it is not immediately obvious.

Unknown History: If you cannot verify the maintenance or usage history of a harness, it must be retired.
Final Thoughts: The Mindset Shift
"Beyond the visual check" is a mindset. It is the transition from "inspecting to find nothing wrong" to "inspecting to find the one thing that could fail."
At your next tool-box talk, don't just talk about how to inspect - talk about why. Remind your crew that their life is attached to those few ounces of webbing. If there is any doubt about the integrity of the gear, the only safe option is to tag it out and remove it from service.
